Polymorphism of alpha-1-antitrypsin and association of S and Z alleles with duodenal ulcers

چکیده
Background: Duodenal ulcers are mucosal erosions that penetrate into the muscularis propria of the duodenum. They are a result of an imbalance between aggressive and defensive factors. Various environmental factors like Helicobacter pylori infection, addictions to smoking and alcohol etc. and genetic factors have been reported to be associated with duodenal ulceration. Alpha-1-antitrypsin was studied for its role as a genetic marker and specific allelic association to protein functioning and alteration. Serum samples from 185 normal subjects and 210 duodenal ulcer cases were typed for the phenotypes following PAGE (polyacrylamide gel electrophoresis) and immunofixation using specific commercial antisera with appropriate staining protocols. In general, ‘M’ allele of alpha-1-antitrypsin was found to be predominant in healthy normal subjects, with the gene frequencies being 0.679 (M), 0.299 (Z) and 0.0214 (S). Whereas in duodenal ulcer cases, Z and S alleles were found to be predominant with a significant association of MS, ZZ and MZ phenotypes (χ2: 49.98) and the gene frequencies being 0. 113 (M), 0. 347 (Z) and 0.506 (S). Predominance of Z and S alleles indicates that these alleles may encode for reduced synthesis of alpha-1-antitrypsin, hence decreased neutralization of proteases like trypsin and chymotrypsin inhibited by alpha-1-antitrypsin, thereby resulting in ulcers. The study highlights the association of Z and S alleles of the potent protease inhibitor alpha-1-antitrypsin and also suggests its role as a genetic marker in ulcerogenesis.

Alpha 1-antitrypsin (AAT) or alpha 1-protease inhibitor (PI) is the principal inhibitor of proteolytic enzyme in serum. Its phenotypic variability has been reported to be associated with liver, lung diseases and rheumatoid arthritis in humans. There is much documentation about high risk phenotypes of PI in some regions of the world, however, there are no reliable reports on these phenotypes and...
متن کاملLow serum alpha 1 antitrypsin in duodenal ulcer--a family study.
Genetic association of a disease is often recognized due to its occurrence in families. Duodenal ulcer like many common diseases has a variable age of onset, expression and familial aggregation . Three lines of evidence support a genetic role: family studies, twin studies and blood group studies. Family aggregation is commonly associated with early onset (<30 years) and blood group 0 with late ...

Serum pepsinogen, alpha 1-antitrypsin (alpha 1-AT) and blood groups were studied as genetic markers in 32 patients with endoscopically proven duodenal ulcer and 44 control subjects with no family history of ulcer disease. Serum pepsinogen was determined by the modified method of Edward et al, alpha 1-AT by single radial immunodiffusion (RID) and phenotyping was carried out by isoelectric focusi...
متن کاملIs Pi a selectively balanced polymorphism?
A stable polymorphism of the Pi alleles M1, M2 and M3 close to their observed frequencies has been predicted, simply by relating fitness to the proportion of each genotype having alpha 1-antitrypsin (alpha 1-AT) levels greater than 100 mg/100 ml. By further specifying that the rare S and Z genotypes with low alpha 1-AT levels have enhanced fertility, equilibria of the M, S and Z alleles have al...
